Interdisciplinary teams provide equitable health care
Health-care professionals working together to support patient care is at the heart of the SHA’s CARES values of compassion, accountability, respect, equity and safety.
For example, Elizabeth and Bonnie are both primary health care pharmacists in Regina on the Intermediate Health Care Services team, an interdisciplinary team that goes out into the community to address complex health issues.
“Individuals who face barriers to care due to things like mobility issues, safety factors, or who live in rural communities really benefit from this service,” said Elizabeth. “This eliminates barriers to ensure equitable health care”.
Their team is made up of pharmacists, paramedics, nurse practitioners, nurses, and social workers who all work together to provide patients with compassionate, respectful, equitable, and safe care.
In addition to this team, Elizabeth and Bonnie provide services to Home Care clients and are connected with primary health-care clinics in Regina, such as the Gardens Community Health Centre, the Four Directions Primary Health Care Clinic and the Meadow Primary Health Care Centre.
“An interdisciplinary team approach to health care is incredible,” said Bonnie. “Having access to a network of health-care providers enables us to enhance care for patients by connecting them with the resources they need.”
The goal of the Intermediate Health Care Services team is to build relationships with patients, their families, and their community providers, to help patients achieve their health goals, and then connect them back to their regular providers. This reduces hospitalizations and optimizes medical care.
